Lasting Power of Attorney (LPA) & Advance Medical Directive (AMD)

Plan ahead with peace of mind through thoughtful healthcare and personal planning.

From Lasting Power of Attorney (LPA) certification to Advance Medical Directive (AMD) discussions, we support patients and families in making informed decisions for the future.

A Lasting Power of Attorney (LPA) is a legal document that lets you appoint one or more trusted persons to make decisions relating to your personal welfare and/or property and financial matters should you become unable to make decisions independently.

As part of the process, a doctor is required to certify that you understand the document and are making the decision voluntarily with mental capacity.
